How the iPledge Program Works in 2026
The program has recently been updated to be more patient-friendly, but it still requires strict adherence to a monthly schedule. Here is the step-by-step breakdown:
1. The Initial Virtual Consult & Enrollment
Your journey begins at Allme with an initial online consultation. During this visit, we will obtain informed consent to register you in the iPLEDGE system.
Initial Consultation Fee: $175
Initial Lab Work: If you are a person of childbearing potential, you will need a baseline pregnancy test (performed in a CLIA-approved lab) and lab work to check your liver function and lipid levels.
If you are not a person of childbearing potential (i.e. were born male, had a tubal ligation, or a hysterectomy), then you will only need liver function and lipid panel tests.
2. The 30-day Waiting Period (For patients who can get pregnant)
FDA rules require a 30-day waiting period between your first negative pregnancy test and your second one. You cannot receive your first prescription until this “qualification” period is complete. If you cannot get pregnant
3. Monthly “Windows” and Requirements
Once you are cleared to start, the “Monthly Window” begins. Each month, you must:
See your provider: At Allme, our virtual monthly follow-up visits are $60. The cost of isotretinoin is determined by your pharmacy. If you have health insurance, you can use that to obtain your prescription.
Get labs done: Perform a monthly pregnancy test. The first two monthly pregnancy tests must be done in a CLIA-approved lab. (Note: Under 2026 guidelines, your provider may allow at-home testing after the second month).
Answer Questions: Log into the iPLEDGE website to answer a few brief comprehension questions. Your provider will give you your iPLEDGE number and you will make a log in directly on the iPLEDGE website.
The 7-Day Window: Once your provider confirms you in the system, you have 7 days to do your comprehension questions and pick up your prescription from the pharmacy. If you miss this window, the system “locks,” and you may need to repeat your lab work.
